Sliding Fee Scale Discount Program. Northland community Health Center provides services to all patients without regard to the patients ability to pay. The SFS program applies a discount to the cost of the services, allowing individual's access to affordable, quality health, dental and prescription services. Qualifications for a discount is based on household size, gross weekly or annual income and insurance/assistance coverage. Verification of income is required. Example: A family of 4 earning $23,800 per year would qualify for a 60% SFS discount.
340B Drug Program. Patients who qualify for SFS discount program may be eligible for the 340 Drug Program. This program allows our patients to receive a discount on certain medications thru our contracted pharmacies. Your discount will be the same as your SFS discount with a minimum fee of $7.00, which is owed by the patient.
Dental Voucher Program. Northland Community Health Center believes that dental services are an important part of primary care services and that this service should be provided without regard to the patients ability to pay. Patients eligible for a dental voucher(discount) must first meet the requirements established in the SFS policy. The patient must inquire for a dental service voucher. Services are limited to those dentists who have agreed to accept NCHC's Voucher.

The new Rolette Community Care Center will have special features: All one level, Fenced in park area, 20 bed Alzheimer Care Unit.
